UCP Programme – April newsletter

The UCP programme team are feeling very regal this month!

Our technology partner, Better, has been awarded Digital Transformation Project of the Year by Health Tech Digital Awards for the work they did to deploy our Universal Care Plan across London in only seven months.  Read more

Even better, we had an all-time high of 3,068 care plans created in March, compared to 1,918 in February.

We have also started working with 12 clinical networks to include condition specific care plan fields and logic as we evolve the UCP to provide personalised care and support plans for a wider range of use cases.

We are hoping to release the next version of the UCP towards the end of the year. Keep an eye on the newsletter for regular updates and opportunities to contribute.

Launch of the UCP Professional Dashboard (Worklist functionality)

Worklists within the UCP are scheduled to go live in June. The Worklists functionality enables web portal users to track patients, activities and tasks around their urgent care plans at user level or workplace level.

Worklists can be built on user level, by displaying patients that have been marked as ‘My patient’ or on an organisation level, where the system displays patients that are assigned to a user’s organisation.

The new Worklists functionality makes keeping track of and managing the care plans of your patients, and your organisation’s patients, easier. More information including a full user guide can be found here.

UCP Business Intelligence Dashboard

The UCP Business Intelligence Dashboard has now been published. Currently it is accessible to UCP ICB leads but access will be given more widely as users become more familiar with what the data means and how to interpret it.  

The UCP Standard Report provides a pan-London view of the key metrics and narrative such as number of new care plans within each borough or ICB; the percentage of people achieving their preferred place of death; and urgent care views. Click here to view the standard monthly report.

The National Record Locator

We are in the final stages of integrating the UCP with the NRL service. This means that Londoners with a UCP care plan will have their preferences and wishes of care accessible to urgent care services regardless of location in England. The NRL integration is projected to go-live this summer.  

OneLondon 5th anniversary

This month marks OneLondon’s 5th anniversary. To mark this milestone they have produced material setting out their work with partners over the last five years to join up information to improve health and care across the Capital and beyond.

We’d like to congratulate London Ambulance Service for an overall 21% increase in UCP views since last August.  The UCP really does offer so much potential to support joined up and effective care across the Capital, and it’s clear that paramedics are seeing the benefit of improved integration with the UCP.
