In November the UCP team attended the HSJ awards ceremony as a very proud finalist in the Driving Efficiency Through Technology category. The team has worked very hard since the launch of the UCP and continues to deliver innovative shared personalised care planning across the capital.
NHS App integration
But there’s more than one reason to celebrate!
We are delighted to announce that the UCP is now live on the NHS App for patients to view across 20 pilot GP surgeries. Please get in touch if you would like to know which practices are involved in the soft launch.
The integration will be rolled out London-wide to all GP practices in the early part of 2024. This will increase visibility of patients wishes and preferences and improve transparency between clinicians, patients and carers.
In other news, the NHS App is being updated, the homepage will be more streamlined and intuitive and the language clearer for patients. Patients can access help and support by by visiting nhs.uk/helpmeapp.
Business Intelligence Dashboard
After some temporary downtime, the BI dashboard is now live and can be accessed, subject to registration, via the Resources page on our website. If you have already registered for an account simply click on the link above to gain access.
Message from Nick Tigere, Head of Programme – Universal Care Plan:
As the year draws to a close, we would like to reflect on our achievements, challenges and plans for the coming year. During 2023, the programme prioritised a set of agreed objectives aimed at providing users with a stable platform since go-live and making select improvements along the way. As the implementation phase is nearing completion, we are now looking ahead to the next few years. But before I share our vision for the future, here are some of the year’s highlights:
- No reported UCP clinical incidents
- Established our People with Lived Experience Group which has 6 active members who act as an extension of the UCP team
- Care plan adoption – utilisation surpassing the previous service in all areas
- Development and implementation of the UCP Professional Dashboard (worklist)
- Development and implementation of the UCP Business Intelligence Dashboard
- Launching UCP in the NHS App and NHS web browser
- Template development (archetypes and data models creation) ready to support new clinical pathways
- Updating our website
- Developing new and exciting training resources

The programme has also faced challenges, namely with the Valida client integration with primary care systems, which have now been resolved. Well done!
This past year we have worked in collaboration with London clinical networks, who have recognised the UCP as a key digital enabler for clinical transformation, identifying several pathways where the UCP can be adopted.
Whilst this is exciting, as we look ahead, we want to continue to engage with ICSs/ICBs and involve system transformation leads.
To this end, we have set up a series of meetings with the ICB/ICS leadership teams to help us understand what to prioritise in order to better support local transformation. The outputs from these sessions will be a co-created set of priorities to inform the UCP roadmap.
We have also taken steps to strengthen our programme governance by aligning with OneLondon and contributing to their Board. Next year is sure to be another exciting one for the UCP, as we continue to deliver a shared personalised care planning solution for London.
