Update on the upcoming UCP expansion
The development of the new, expanded UCP template is now complete and it is currently undergoing rigorous testing to ensure it’s clinically safe. We are also making sure that all submitted data in existing care plans will seamlessly transfer to the expanded template.
To help you prepare for the change, we are creating a range of resources that will be available soon on the learn page of our website. These will include:
- A 10-minute training video highlighting key functionalities and providing an overview of the content.
- A quick reference guide outlining the contents of each form in the care plan.
- The training portal will be updated to reflect the ‘final’ version of the expanded UCP so users can familiarise themselves with it.
- Case studies that can be used to create example care plans on the training portal.

Please submit all in progress forms ahead of the UCP expansion
Our Senior Clinical Transformation Manager, Tomas Ince, wanted to share an important message and story ahead of the UCP expansion.
“Last week, I spoke with a nurse about a fantastic UCP conversation she recently had with a patient. She documented this in the UCP, which I was naturally very pleased about. However, she mentioned that because the discussion was ongoing, she did not submit the forms. I quickly reminded her that unsubmitted forms are not viewable by urgent care services in the summary meaning they are less likely to act on any recommendations made. She was surprised and now understands that the UCP evolves during the life of a patient and therefore submitting forms provides the most accurate information about what matters to that person at that moment in time.
In addition to this, the UCP is expanding and as our key messages illustrate all submitted data will be migrated to the new template and no care plans will be lost. However, Forms in the care plan in an ‘in progress’ state may not be migrated. Let me explain; In the current UCP, diagnosis and prognosis data fields are in different forms. In the expanded UCP template, they have been brought together into the same form called ‘Diagnosis and Prognosis’ to make the UCP more intuitive to use. If you have submitted the diagnosis form but have the prognosis form ‘in progress’, when they are merged only the submitted diagnosis form will be migrated to the new template. If both are submitted, all information will migrate to the new template. Therefore, it is essential that all completed forms are submitted before the launch of the new template planned for December 2024.”

NEMs integration is now live
We have recently launched an integration with the NHSE National Event Management Service (NEMS). The UCP system now receives notifications when a patient’s deceased status updates on the NHSE Patient Demographics Service (PDS), allowing the UCP to automatically update the UCP death status in the background.
Our clinical leads encourage clinicians to continue completing post death audits. Post death audits are vital in helping services improve and work towards achieving better outcomes for patients. IE11 Deprecation
The UCP will no longer support the Internet Explorer 11 (IE11) browser from December due to security reasons. Users whose clinical application uses IE11 will see a message when they are viewing care plans in the UCP. At this stage, there is no action required as we are already working with London trusts and organisations to resolve any browser upgrades.

BI Dashboard is back up and running
We are pleased to inform you that the issue with the BI dashboard has been resolved, and it’s now fully operational again.
The dashboard allows users to view UCP utilisation data across London, providing insights into how and by whom the UCP is used. All current London organisations with access to the UCP system are approved for access to the BI Dashboard.

UCP People with Lived Experience Group
Thank you to the members of the People with Lived Experience Group (PLEG) who met with us this week.
Members of the PLEG have either been cared for or supported someone who has been cared for by several health and care services. They are vital, ensuring that the patient voice remains at the centre of the design process as the UCP evolves.
