On 18th September, we hosted a care home learn and share event, bringing together care home support teams and care home managers to share what’s been working well, and discuss the challenges in supporting care homes to adopt and use the UCP.
Thank you to everyone who joined, we had an amazing response with over 90 participants!
We’re so grateful for everyone’s engagement. Attendees shared thoughtful ideas, honest challenges, and creative solutions for making the UCP part of everyday practice in care homes.

Who Joined Us
We began the session by learning more about who joined the session. Knowledge of the UCP varied among attendees – some were already very familiar with it, while others were new to it. Around 45% of participants worked in a care home, and the majority were based in South London.

Attendees were also asked to share one word that described their experience with the UCP so far. The most common words were ‘useful‘, ‘confidence‘ and ‘communication‘.

Breakout Rooms – What’s Working Well
In breakout rooms, attendees discussed what’s been working well. Key themes included:
- Emergency preparedness and DNACPRs
- Personalisation and resident centred care
- Information sharing & multidisciplinary team collaboration
- Confidence and staff empowerment
- Admission avoidance & continuity of care

Breakout Rooms – Addressing Challenges Together
Attendees then explored the challenges. The most common issues raised were:
- Access and system integration
- Quality of information
- Relatives’ trust in the system
- Training and implementation
They discussed potential solutions and ways forward. The key points on challenges and how they can be addressed together are summarised in the images below.

Session Feedback
At the end of the session, attendees were asked what types of events they’d like in future. Most wanted a mix of similar sessions and training opportunities.
