Universal Care Plan News – November 2025

Last month, the UCP went live with a GP Connect (Access: Structured Record) integration. This enables organisations with access to GP Connect to view medication and allergy information recorded in the patient’s GP record when in the UCP, helping reduce discrepancies and the administrative burden.

After the next UCP system update, users will be able to add an alert that the person has a catheter in the ‘Alerts’ form. This will help urgent care services to assess and treat the person safely and effectively in emergency situations and support better communication between services. 


Did you know a staff member’s ability to edit the UCP depends on their role?

Everyone can view the full care plan, but role-based access controls (RBAC) determine what they can edit. Non-clinical staff can only edit certain forms within the care plan.

The average number of care plan views per month by urgent care services have increased by 29.7% so far in this financial year compared to 24/25 financial year. It’s positive to see the growth in usage, enabling more and more opportunities for care to be personalised in urgent care situations.


Following feedback from our recent Care Home Learn and Share event, we’re organising two sessions to further support the use of the UCP across care settings.

UCP Access and Training for Care Home Staff: Getting Started with the UCP 

On 25th November we’re holding a webinar that will provide practical, step-by-step guidance to help care home staff get started with the UCP. The session will cover how to gain access and navigate the system effectively. It’s designed for care home staff who are aware of the UCP but would like help with accessing and beginning to use it. Hospice Learn and Share Session

We’re also planning to hold a Learn and Share event focused on the UCP in hospices. This session will build on insights from the care home discussions, exploring both challenges and examples of what’s worked well. A ‘how to’ guide for carer contingency planning has been developed which includes information on: ​

  • How to create and view a carer contingency plan (CCP) in the UCP ​
  • How you can support carers to start their own CCP on the UCP ​
  • An example of what a CCP can look like in the UCP
