Thank you to our People with Lived Experience Group & Champions 

Last month we met with our UCP Champions and People with Lived Experience Group (PLEG). Thank you to everyone who was able to attend the meetings! 

Our champions group help us to raise awareness of the UCP. If you would like to be part of the group, please get in touch. Members of the PLEG have either been cared for or supported someone who has been cared for by several health and care services and help to ensure that the patient voice remains at the centre of the design process as the UCP evolves. 


Highest UCP utilisation by care homes in February 

February saw the highest number of logins and care plans accessed by care home staff since the UCP launched. Staff viewed over 500 care plans – a significant increase from 125 in February 2024. User logins also increased more than 5 times compared to the same time last year. Thank you to everyone who has contributed to this.

Three quarters of Londoners with sickle cell disease have a UCP

March marked one year since the UCP expanded to support people with sickle cell disease (SCD). Now more than three quarters of people with SCD who receive care in London have a UCP – an incredible achievement!

UCP expansion: new forms supporting better care

As previously communicated, the UCP expansion introduced four new forms to the care plan: 

  • What matters to them – focuses on personalised care
  • Daily activities and support needs – information on the person’s support needs and who provides this
  • Communication and accessibility – captures communication needs and reasonable adjustments
  • Medical devices – information about any devices currently in use to support care

It has been great to see that since the expansion in January, these forms have been completed thousands of times. Thank you to everyone who has contributed to this!
