UCP Editable Access for Londoners: Launch Update and April Deadline
UCP editable access for Londoners remains planned to go live in late spring. This will enable members of the public to start their care plan and edit selected personal information in real time within the UCP, through the NHS App and online login service.
There is now under a week remaining to review and submit any forms saved as ‘in progress’. From 14th April, the functionality to save six forms as ‘in progress’ will be removed, and any forms still in this status will be removed from the system. Please ensure all relevant information is submitted before this date. Find out more here.
When editable access for Londoners goes live, it will be available to all users across London. However, we will initially focus on supporting a group of early adopter GP practices. This approach will help us gather insights and refine support ahead of a wider rollout.

UCP Editable Access for Londoners: Launch Update and April Deadline
This month we’re highlighting the South West London (SWL) Care Home Digital Integration team and their work to support care homes implement the UCP.
Harry Dawson, Senior Health & Social Care Integration Officer, shares an overview of their approach and impact:
We’ve taken a personalised approach, offering dedicated one‑to‑one support for care home managers and colleagues, and building confidence in using the UCP in day‑to‑day delivery of care.
In 2023, only 6% of care homes in SWL had access to the UCP. As of 2026, this has risen to 62%.
To achieve this, we’ve focused on proactive engagement and regularly reviewed usage data to identify homes that may need additional support. Working closely with key stakeholders including GP practices, care home support teams, hospices and SWL colleagues, has been key to encouraging consistent use across the system.
To celebrate the progress made in digital adoption, we hosted an event showcasing achievements across SWL and explored what’s next for digital innovation in care homes.
Our ongoing priority is to sustain engagement, improve consistency of use, and ensure the UCP is embedded as a core tool for delivering high quality, coordinated care.
Ajitha, Care Home Manager at The White House, shared:
This has created a bridge between the home and the other professionals which supports in continuing with the wishes of the resident. We are able to fullfill the preferences of the resident by developing the UCP.
The discussion with the resident and family brings a lot of information to be able to support them. This helps the emergency services to follow the UCP and there’s no room for confusion.

UCP Editable Access for Londoners: Launch Update and April Deadline

On 3rd March, we held a learn and share session focused on the UCP and palliative and end of life care (PEoLC), bringing together professionals supporting the PEoLC population in London to share UCP programme updates and provide an opportunity to share feedback.
Thank you to everyone who joined, it was fantastic to see so many people come together for this session. We’re grateful for the continued engagement and valuable insights shared during the session.

UCP Report
Log-ins remain high, with over 60,000 total log-ins recorded for the second consecutive month. Unique log-ins have also remained high, exceeding 11,000 for the third consecutive month.

UCP at Digital Health Rewired
We were pleased to be part of the OneLondon programme showcase at the Shared Care Record Summit that took place at Digital Health Rewired in Birmingham at the end of March.
Tomas Ince, UCP Senior Clinical Transformation Manager, spoke during a session on patient engagement about the upcoming editable access for Londoners functionality.
He said:
‘It was encouraging to see strong alignment nationally around the shift towards a Single Patient Record, utilising current digital infrastructure. For us, enabling patients to contribute directly to their UCP is a key step in making that vision meaningful, turning shared records into truly personalised, co-created care plans’

Dates for the Diary
14th April – Review and submit ‘in progress’ forms before this date as ‘in progress’ functionality will be removed for 6 forms and any information still saved as ‘in progress’ in these forms will be removed from the system.
