There are exciting times ahead for the UCP with the expanded template launch planned for 10th December this year! Currently, we are still testing the new template and so the launch date is subject to change whilst we make sure it is clinically safe.

You will now see a notification in the live system informing users of the upcoming change. As we get closer to the planned launch date, we would like to remind clinicians to please submit any forms that are in progress. All submitted data will be migrated to the new template and no care plans will be lost. However, forms in the care plan that are in an ‘in progress’ state may not be migrated.

The learn page of our website has also been refreshed with updated and new resources. These resources have been organised into three sections: ‘got 5 minutes?’, ‘time for lunch?’ and ‘protected learning time’. This ensures you have access to just enough information, just when you need it.

If you would like to see the expanded template before the launch, the training portal will be updated soon to reflect the ‘final’ version so users can familiarise themselves with it. You can log in or sign up for access to the training portal here

Additionally, you can register for the two webinars we are hosting on Wednesday 27th November and Thursday 5th December to explain how the changes will affect current users and to answer any questions you may have.

You can always find the key messages about the UCP expansion on our website here.

IE11 Deprecation

The UCP will no longer support the Internet Explorer 11 (IE11) browser from December due to security reasons. Users whose clinical application uses IE11 will shortly see a new message when they are viewing care plans in the UCP, informing them that from early December they will no longer be able to access the UCP through IE11.

If you see this message and you have not been contacted by your organisation about how to access the UCP after December, please contact your local IT helpdesk. Please note, this does not apply to users from Imperial Trust and St George’s Trust as we are already working with your organisations.

The UCP team are collaborating with The Health Innovation Network (HIN) South London on a project aimed at increasing the awareness and adoption of the UCP in care homes across London.

Throughout September, the HIN team gathered insights from across the system and are using this knowledge to start the development of a suite of resources. These resources will be aimed at care home staff to support them to sign up, access and use the UCP. Some examples of what is in development are; a co-produced information leaflet about the UCP for residents and carers, posters for promotion in care homes, a video featuring care homes staff speaking about their experience of using the UCP, and a guidance document of how to use the plan together with the benefits of doing so.

Feedback from care home staff using the UCP has been very positive, as shown by these quotes from two homes in South West London:

“One of the most valuable tools we are using is the UCP, which has provided a wonderful platform to highlight the preferences of our residents…We are incredibly grateful for the support from all the health professionals who are assisting us in populating this tool.”
— Manager, SWL Care Home

“This has created a bridge between the home and other professionals, allowing us to uphold the wishes of our residents. Through the UCP, we can better fulfil our residents’ preferences.”
— Manager, SWL Care Home

If you would like to find out more information about the resources being created, or you work in a care home and want to be involved please get in touch

The UCP team have been working hard to develop a clinical strategy for the next few years, undertaking clinical engagement with regional stakeholders including patient representatives, clinicians, clinical network leaders, integrated care boards and urgent care services. We’ve held 7 listening workshops and received over 120 survey responses from the public.

Thank you for all those that have engaged with the clinical strategy. Our conversations have been rich, full of energy and hope for the future of the programme. We look forward to sharing further information about the strategy in the new year.

A couple of months ago, we welcomed a new team member! Sara has joined us as our Project and Communications Officer. In this role, she will be supporting our clinical transformation efforts and will work closely with our champions and People with Lived Experience Group (PLEG).

Sara brings valuable experience from her previous work in supporting digital implementation in care homes. She shared that she’s thrilled to join the UCP team during this exciting period of transformation.
