Milestone achieved: Over 100,000 UCPs created!

Since the UCP went live, more than 100,000 care plans have been created enabling health and care professionals across London to share vital information about their patients and to deliver more personalised, coordinated care.
This milestone reflects the dedication of colleagues across health and social care who create, update, and use care plans to help improve outcomes for people.

Ageing Well Spotlight
This month, we’re highlighting some of the ongoing work to increase awareness and adoption of the UCP, as part of supporting people to age well.
The Ageing Well Clinical Network has developed a tailored version of our communications toolkit to support teams working with older people. You can download it here.
We also spoke with Goldie Bacuyani, Matron for Older People and Dementia Services at London North West Hospital (LNWH) and UCP Champion, who described how her team is working with patients, carers, and staff to put the UCP into practice:
The Dementia and Older People’s Team at LNWH is focused on improving services and the hospital experience for individuals living with dementia, delirium, or cognitive impairment, as well as for their carers. Part of this includes promoting and creating UCPs to help support person-centred, compassionate care and ensure that the patients’ needs and preferences are respected – even when they are unable to communicate them.
We highlight the UCP at our monthly Dementia Hub, where we work closely with community partners to engage families, visitors, unpaid carers, staff, and patients. These sessions aim to raise awareness of the UCP and give staff practical guidance on how to access and update it.
As part of Falls Awareness Week in September, we will have a UCP stall to show how it can support person-centred care and falls management among older patients.
We will also be piloting a monthly training programme for volunteers and unpaid carers across Harrow, Brent, and Ealing. A key element of this training will be completing UCPs, with the aim of increasing awareness and uptake in the community.
The Universal Care Plan support us with advance digital care planning and actively involve carers in the development of contingency plans. It is a valuable resource for staff, helping them to identify and implement reasonable adjustments for patients with communication needs.
One patient shared his appreciation with our Activity Coordinator after they created his UCP within our Electronic Patient Record:
“Thank you for asking me these questions now. I am happy that I don’t have to repeat myself again and again when I come to my GP or the hospital.”

Meet Team OneLondon – Yomi Ogunsola, a UCP Champion
The latest Meet Team OneLondon features one of our UCP Champions, Yomi Ogunsola, who is the Interim Care Home Quality Nurse at South East London ICB (Bromley).

UCP Monthly Report
There were over 40,000 total log-ins in July from almost 10,000 unique users.

UCP Adoption for Sickle Cell Patients in South London
South London’s Office of Specialised Services’ annual report shows that 5,540 UCPs are now live for people with Sickle Cell Disease – covering 92.3% of the local Sickle Cell population. You can read the full report here.
