Universal Care Plan Palliative and End of Life Care Learn and Share Session – Key Learnings March 2026

On 3rd March, we held a learn and share session focused on the UCP and palliative and end of life care (PEoLC), bringing together professionals supporting the PEoLC population in London to share UCP Programme updates and provide an opportunity to share feedback.

The session also included a panel discussion on topics including how to get started with the UCP, tips for engagement campaigns, what paramedics and acute services look for in a UCP, and how it supports patient care.

We had 201 attendees (!) so thank you to everyone who joined, we’re grateful for the continued engagement and valuable insights shared during the session.

We began the feedback portion of the session by asking how often attendees currently use the UCP. There was a range of responses with 55% of attendees using it either frequently or very frequently in a typical week.

We asked attendees how they find the UCP useful in their role. The most common themes were recording patient wishes, sharing person-centred information, and supporting DNACPR decisions. When asked which information in the UCP they find most helpful, similar themes emerged, with escalation plans also highlighted as particularly valuable.

Attendees then fed back on some of the challenges they can experience. The most common barriers were time pressures, access issues and a lack of information.

We also asked attendees to share the support that would help them overcome these challenges and use the UCP effectively. The most common suggestions were improved access, more training and awareness and more access by care homes. This feedback will help the UCP Programme to focus our support, where we can, in some of these areas.
