One month on from the UCP expansion

It’s been over a month since the UCP care plan expanded to become a broader personalised care and support plan, helping us to reach our motto of ‘What matters to me is just as important as what’s the matter with me’. So far, we’ve seen strong engagement with usage levels remaining consistent with pre-expansion data, and we’ve received positive feedback from health and social care professionals on the new forms.

This isn’t the final destination for the UCP. Together with our clinical and ICB stakeholders, we plan to continue developing the care plan, so that it meets the region’s clinical priorities. In the near future, we’re focusing on introducing editable access for patients via the NHS App. While patients have been able to view their UCP via the NHS App since March 2024, we are working hard to enable patients to enter important details about themselves including personal preferences and what matters to them. This will provide health and social care professionals with further insights into how the person wishes to be assessed and treated.

UCP Monthly Report 

January was a fantastic month for the UCP! We saw a record number of unique logins and a strong increase in new care plan creations.


Marking one year of UCP support for sickle cell disease

On 14th March, we will mark one year since the UCP expanded to support people with sickle cell disease, enabling clinicians to record specific directives for paramedics, emergency departments, and wards to help care for someone experiencing a pain crisis within the UCP. 

There are now over 7,700 people with sickle cell disease who have a UCP – approximately 74% of those receiving treatment in London. Since March 2024, these plans have been viewed 3,205 times by urgent care services, a significant increase from 570 patients in the same period the previous year. A huge thank you to everyone who has contributed to this progress!
