We have collaborated with the Health Innovation Network (HIN) South London to increase UCP awareness in care homes across London and look at ways we can enable care home staff to use the UCP.
As part of this work, the HIN team has gathered valuable insights from a variety of stakeholders and the following resources have been co-developed specifically to address barriers and support care home staff.

- In depth care home guidance document to help care home staff access/use the UCP and outlining where to find help if required
- Posters care homes can print and display to raise awareness to staff and visitors that their care home uses the UCP
- Benefits documents outlining the benefits of using the UCP for care home staff, residents, and families, as well as for the wider health and care system
- Testimonials from care home staff discussing how using the UCP has benefitted their care home
- Resources for care home residents and their family explaining what the UCP, why it is used and the benefits to support conversations about advance care planning and preparing a UCP
- A video featuring care home staff, the London Ambulance Service, a care home matron and GP sharing their experiences of using the UCP.
Did you know a staff member’s ability to edit the UCP depends on their role?
Everyone can view the full care plan, but role-based access controls (RBAC) determine what they can edit. Non-clinical staff can only edit certain forms within the care plan. View and print a guide on how this works here.
